Medication guide:

Rifampin is a medicine from the class of antibiotics. It makes a part of rifampicin group of antibiotics aimed at the treatment of tuberculosis, leprosy, abscesses, meningococcal disease, and prevention Haemophilus influenzae type b.

The group of bacteria that shown ability to fight  previously uncured illnesses such as tuberculosis was found in 1957 in samples of soil from French pine forest. The researchers who discovered curing properties of the bacteria named it in honor of their favorite movie “Rififi”. After years of work on the bacteria stabilization, the medicine was first used in 1971.

owadays, it is widely used in a number of medicines with similar effect mostly in combination therapy because as a single-therapy drug Rifampin and other identical medicines are known to develop resistance in bacteria after short period of usage.

Chemical compound and properties of Rifampin

The medicine is based on a chemical formula C43H58N4O12. It is represents a crystalline powder of brown-red color which is not very soluble in water but is perfectly soluble in chloroform and other solutions with less neutral pH than water.

Rifampin – forms of supply

Rifampin can be prescribed either in capsules of 150 mg or 300 mg or intravenously through a drip. When prescribed intravenously the medicine should be administered by a medical worker in a hospital or at home. For the intravenous administration the medicine comes in 600 milligram vials with the solution of the drug with additional components to make it compatible with the blood pH.

Rifampin - Dosages

The dosage of Rifampin is chosen for every patient individually. It is calculated based on a patient weight. Usually, 10 milligrams of the medicine per kilogram of a patient’s body weight are prescribed. The maximum daily dose must not exceed 600 milligrams a day.

It is recommended to use the medicine once a day, preferably on an empty stomach, and drink a full glass (200 ml or 8 oz.) of water with it. Food can reduce the absorption of the medicine by 70% reducing its efficacy respectively.

Taking into account that the medicine is commonly prescribed in combination therapy with other medicines to prevent bacterial resistance to the treatment, it is preferable to take the medicine in the morning and wait at least 12 hours to take other antibiotics that can lessen its effect. However, the dosage and recommended time of use are prescribed by a doctor for each case individually.

Rifampin - Pharmacokinetics

As provided above, Rifampin is supplied in capsules for oral use or in solution for intravenous administration. When the medicine is taken orally, the peak concentration in plasma during the clinical trials was observed from two to four hours after the intake. Meals before or no more than an hour after Rifampin use significantly reduce the peak concentration and overall action of the medicine.

The medicine is almost completely excreted from the body – 30% comes out with urine and from 60% to 65% with feces. The color of urine can indicate the quality of the medicine absorption in the body – the more intense color the urine has after the medicine intake, the lower dosage of Rifampin was absorbed.

When the medicine is administered intravenously, its peak concentration in plasma is believed to be achieved after the thirty-minute infusion immediately as the medicine enters directly into the blood flow.

Rifampin - Mechanism of action

The medicine acts physically on the synthesis of bacteria RNA. After the administration, it attaches to the bacteria RNA polymerase blocking the phosphodiester bond in its backbone.

When the medicine is contraindicated

The medicine is contraindicated to people with individual intolerance of rifampicines. It is also not recommended to use the medicine with certain antiviral medicines as during the clinical trials it was observed that Rifampin significantly reduces their effectiveness and promotes development of resistance toward the drugs in the viruses subjected to the treatment. The medicines that should not be taken with Rifampin are:

- Darunavir
- Ritonavir-boosted saquinavir
- Atazanavi
- Tipranavir
- Saquinavir
- Fosamprenavir

Important notice: Taking into account that Rifampin and the antiviral medicines listed above can be marketed under the different brand names, it is necessary to consult your doctor before adding any medicine to your treatment plan to be sure that the medicines used do not prevent each other from effective impact on the bacteria and viruses treated and do not harm your health.

Adverse effects of Rifampin

As any other medicine, Rifampin can become a cause of certain unpleasant or even life-threatening conditions. Before using the medicine it is advised to undergo medical tests that will help to identify whether you have the intolerance of its active ingredient and if your liver and kidney are in sufficiently healthy condition for the use of potent antibiotics. It is also important to comprehend to the dosages and modes of administration recommended by a doctor to avoid overdose and subsequent serious side effects that may include:
- anorexia, nausea, cramps,  vomiting,  heartburn, jaundice, epigastric distress, flatulence, and diarrhea, sore mouth, sore tongue, conjunctivitis, urticaria
- liver dysfunction (increased bilirubin, etc.), hepatitis or a shock-like syndrome with hepatic involvement
- cerebral hemorrhage, thrombocytopenia, leukopenia, decreased hemoglobin, and hemolytic anemia,
- headache, fever, violation of sleep cycles, fatigue, ataxia,  dizziness, mental confusion, inability to concentrate, behavioral changes, muscular weakness, pains in hands or legs, blurred vision
- adrenal insufficiency, menstrual disturbances,
-  rash, flushing, itching
- hemolysis, hematuria, interstitial nephritis, acute hemoglobinuria, renal insufficiency, tubular necrosis, and acute renal failure and others.

Generic Rifampin

Rifampin is a powerful antibiotic used in the treatment of such diseases as tuberculosis, leprosy and others if your doctor thinks that the disease you are suffering from is caused by the bacteria sensitive to the main active component of the medicine.

The drug was first used in 1971 after numerous clinical trials following the discovery of “magic” bacteria found in pine forest soil able to destroy life-threatening bacteria in human. Nowadays, it is widely used for serious, chronic bacterial diseases in combination therapy with pyrazinamide, isoniazid, ethambutol, and streptomycin. The named drugs are also antibiotics that are used to prevent the bacteria, which are being eliminated, from the development of resistance to Rifampin or other medicine with the same composition but other brand name.

History of the brand name and its variations

Two scientists who first discovered the bacteria, on which all research and the creation of the medicine are based, were fond of a French movie called “Rififi”. They decided to name their find using the part of the movie name. Therefore, nowadays the medicine bears name Rifampin while the group of medicines based on the active ingredient is called rifamycins.

Worth noting that in different countries the medicine can be found under various quite different brand names. For example, in Bulgaria the drug is called Tubocin, in Romania – Sinerdol, in USA and UK – Rifadin, in France – Rifadine, etc. The medicines are identical, but they are manufactured by different companies. Respectively, the medicines are sold at different prices. If you want to buy cheaper Rifampin, you can find its analogs online looking for “Generic Rifampin”.

Generic Rifampin recommended dosage

Unlike many other medicines, the Generic Rifampin dosage does not depend on the age of a patient, but rather on his weight. The daily dosage is calculated as follows: 10 milligrams of the drug per one kilogram of a patient’s body weight. However, the maximum dosage used per day cannot exceed 600 milligrams of Generic Rifampin in order to avoid negative impact on kidneys and liver. For some patients, intravenous infusions are prescribed. The dosage of Generic Rifampin per infusion is also established individually but cannot exceed 600 milligrams per day.

The medicine is advised to be used at least two hours apart from meals because food significantly lowers the absorption of the rifamycins and contributes to reduced effect of the medicine. The medicine is excreted from the body with urine and feces. Most part of Generic Rifampin comes out with feces, but based on the urine color a patient or doctor can assess the level of absorption of the medicine – the more intense is the color, the fewer medicine is absorbed, which is quite a bad sign, meaning that the drug is administered wrongly.

Contraindications and side effects of Generic Rifampin

The medicine can cause a number of side effects similar to other antibiotics: gastrointestinal tract disorders and discomfort, liver function disorders, kidney disorders or dysfunction, neurological manifestations (headache, cramps, etc.), sore mouth, nausea, etc. However, most of the side effects are more likely to occur in individual hypersensitivity to Generic Rifampin or other rifamycins-based drug or in improper use of the medicine. For example, it is strictly not advised to break the treatment course abruptly and then start it again from the same dosage.

Since the intolerance of the medicine not always can be found before its use, it is advised to take Generic Rifampin only under comprehensible doctor supervision.

The medicine is not recommended to be used with certain antiviral medicines because it makes them ineffective and sometimes together they can harm a patient’s health instead of helping to cure the disease.
